Acne fulminans is a sudden onset of highly destructive inflammation, which  normally afflicts young men.  Symptoms of severe nodulocystic, often ulcerating  acne, aching joints and fever are apparent.  It may result from unsuccessful  treatment of acne conglobata.  It does not respond well to antibiotics, so  accutane and oral steroids are normally prescribed.  Corticosteroids or  non-steroidal anti-inflammatory medications may be given to reduce inflammation.  Attacks of acne fulminans may recur, and the patient may develop acne that  requires long-term treatment with isotretinoin.</p>
<p align="justify"><span><strong>Gram-negative</strong></span><strong> folliculitis</strong><br />
Gram-negative folliculitisis is a complication of abusive  and long-term use of antibiotics.  Patients with this type of acne are usually  treated with accutane since it is known to be effective against gram-negative  bacteria. The word “Gram” refers to a blue stain used in laboratories to detect  microscopic organisms. Certain bacteria do not stain blue and are called “Gram  negative.”</p>
<p align="justify"><span><strong>Pyoderma</strong></span><strong> faciale</strong><br />
Pyoderma faciale  is the type of acne that affects females mainly belonging to the 20-40 age  groups. It is confined to the face, appears suddenly as both deep and  superficial cystic lesions of the face with interconnecting "tunnels" between  cysts, reddish inflammation of the skin, and slight swelling (edema) of the  skin, rarely persists more than a year or so, and is not associated with oily  skin.</p>

<p align="justify">Acne is a term for plugged pores (blackheads, whiteheads),  pimples and even deeper lumps (cysts or nodules), that occur on the face, neck,  chest, back, shoulders and even the upper arms.</p>
<p align="justify">There are several variations of acne that skin experts coined.</p>
<p align="justify"><span>Types</span> <span>of acne</span></p>
<p align="justify"><strong>Acne Vulgaris</strong>(common acne) includes several types of  lesions, usually called blackheads, whiteheads, papules, pustules, nodules and  cysts.</p>
<p align="justify"><em>Mild to moderate</em> acne vulgaris is characterized by:</p>
<p align="justify"><strong>Blackheads</strong> are caused by partially blocked pores.  The  black appearance is caused by melanin pigment from dead skin cells. Blackheads  tend to be stable structures, and often take a long time to clear.</p>
<p align="justify"><strong>Whiteheads</strong> are caused by completely blocked pores. The  white appearance is caused by the bacteria inside turning the sebum into free  fatty acid. The whitehead differs in color from the blackhead because the  opening of the plugged sebaceous follicle to the skin’s surface is closed or  very narrow, in contrast to the distended follicular opening of the blackhead.</p>
<p align="justify"><strong>Don'ts:</strong></p>
<p align="justify">Neither blackheads nor whiteheads should be squeezed or picked  open, unless extracted by a dermatologist under sterile conditions. Tissue  injured by squeezing or picking can become infected by staphylococci,  streptococci and other skin bacteria.</p>
<p align="justify"><strong>Papules</strong> are small, red, tender bumps or spots. A group  of very small papules and microcomedones may be almost invisible but have a  "sandpaper" feel to the touch. A papule is caused by localized cellular reaction  to the process of acne.</p>
<p align="justify"><strong>Pustules</strong> are larger, red and inflamed-pus filled spots.</p>
<p align="justify">On the other hand, <em>severe</em> acne vulgaris is characterized  by:</p>
<p align="justify"><strong>Nodules</strong> aare large and hard bumps under the skin’s  surface.  Unlike a papule, a nodule is characterized by inflammation, extends  into deeper layers of the skin and may cause tissue destruction that results in  scarring. A nodule may be very painful.</p>
<p align="justify"><strong>Cysts</strong> are similar to nodules but can be severely  inflamed and infected.</p>
<p align="justify"><strong>Acne Rosacea </strong>is a variant that typically causes a red  nose and is more common in older people than in young people, mostly affecting  those who are aged 30 and above.  It commonly appears as a red rash, which are  normally confined to the cheeks, nose, forehead, and chin. The redness is often  accompanied by bumps and pimples. Blood vessels may also become more visible on  the skin. People with this kind of acne rarely have pimples.  These flashes can  be triggered by certain foods like spicy foods and alcohol.</p>

<content:encoded><![CDATA[<p>Spoke to the hospital this morning and they have decided not to operate on my cataract tomorrow as planned. I was up all night with breathing difficulties and knew in my heart that it wouldn't be safe to go ahead, much as I wanted to. Now someone else can get my place.</p>
<p>I can't tell you how disappointed I am that this has happened. It's not just the length of time I've waited (about a year) but its about clearing the diary, getting cover arranged and all the planning that has gone into getting this op and recovery time sorted. I am angry and upset.</p>
<p>The plan now is that I phone when I am well and they give me another appointment. It could be months away and will involve more diary negotiations.</p>
<p>So, the doctor will see me today and hopefully give me more steroids and this time I shall stay indoors and not work at all until I am well. Bishop Alan will take the service on Sunday as planned.</p>

<content:encoded><![CDATA[<p>So last night I figured I would walk the kids to school this morning, rather than take the car. Save on petrol and get a little exercise in. But on waking  I realized I am way too optimistic in the evenings, no way was I gonna don my trainers and set out on foot. I hadn't even had my morning cuppa coffee! What I have started doing is drinking a cup of apple cider vinegar and honey in hot water when I wake up, while I get the kids ready for school. It's  yummy, and it makes me feel that I am doing something concrete about my health.</p>
<p>My belly measures 87cm round, and I would like to bring that down quite a bit. Not sure what I'm aimiing for in the measurement department, but I would definitely like a reduction of some kind. Today I am cutting down my sugar consumption, only one spoon in my coffee rather than two...every little bit helps!</p>

<p>The skin is the largest vital organ of the human body. It covers you up,  keeps you warm and keeps you cool. It also decides what can be absorbed and what  should be rejected.</p>
<p>Skin that is clogged and unhealthy is not just a beauty problem. It can  become a hindrance to your sense of vitality and wellness. Proper care of your  skin is important not only to your personal sense of beauty but also to proper  elimination, more graceful aging and overall health.</p>
<p><span>The skin has</span> <span>five main functions:</span></p>

<p>The core body temperature needs to be kept constant for normal physiological  activity to take place (37°c). It needs to maintain a core temperature for  homeostasis.</td>

<p><a href="http://www.dailymail.co.uk/news/article-1069603/Gay-men-forced-health-warning-tattoos-says-Stock-Exchange-chaplain.html" target="_blank">And then this guy The Rev Peter Mullen, chaplain to the London Stock Exchange,</a> called for homosexuals to be tattooed with warnings about the perils of gay sex in a cigarette-packet-style health warning. Mr Mullen, 66, has insisted it had all been a joke and he had nothing against gay men. But he was forced to remove the comments from his Internet blog, and the Bishop of London described them as 'highly offensive'. <strong><span style="text-decoration:underline;">I have to agree.</span></strong> Do we put tattoos on wimmin saying "This my damage your wallet", and on men saying "This may leave the toilet seat up." etc etc etc.</p>
<p>And on ALL doctors saying "This man/woman may misdiagnose/mistreat and eventually kill you, but will never explain their actions" Just my opinion considering <a href="http://themaddentist.wordpress.com/2008/08/31/doctor-a/" target="_blank">Dr A</a> and for example <a href="http://themaddentist.wordpress.com/2008/06/02/dr-k-says-get-over-it/" target="_blank">Dr K.</a></p>

<p>I totally despise tattoos, and Governmental Warnings,  and remember a friend of my Dad's showing me his faded and enormous tattoos that he drunkenly got before, after and during the 2nd World War. I was a lickle fat  pre teen and was in TOTAL AWE of this man as he graphically described the horrors of tattoos. A friend now is covered in tats he received from his time in jails and has said the same to me recently. He showed me a few "scribbles" done with a needle and ink in his cell and I do agree. "Where <span style="text-decoration:underline;"><strong>does</strong></span> that snake's head go?? eughh!"</p>
<p>I jokingly wanted a tat on my bypass scar and or the donor site scar, both extremely visible that says the date and the misdiagnosis that caused the need for the op with the Doctor's name.</p>

BUT it would graphically show the dangers of seeing a Psychiatrist. And then give the "normal" people plenty of opportunity to show us even more discrimination. Should Psychiatrists be tattooed with governmental (excuse the pun) warnings?????</p>
<p>SOO by a convoluted route my brain has lead me to think that ALL Doctor's surgeries should have a notice showing the Doctors success and failure rates so that ALL future patients could see just "how good" this surgery was.</p>
<p>I had a poster in the Practice showing the number of appointments that week. The number of patients that failed their appts. The number of Patients that cancelled their appts and then the number of those appts that we were able to fill. The number of fillings, extractions, dentures and crowns were also down on that sheet. It also showed the average time for ,say, making a denture (3 weeks). THANK GOD for computers in the late 1990's. What could I do with one now???</p>
<p>Oh yeh write a blog showing that <a href="http://www.mad-dentist.co.uk" target="_blank">Dr A totally fucked my life</a> and is not prepared to explain his actions. The Welsh Assembly then refuse, my word, to investigate the corruption and the web of deceit and lies woven by Dr A and various many others.</p>

<content:encoded><![CDATA[<p>ช่วงนี้ผู้คนต่างตื่นตระหนกกับข่าวการปนเปื้อนของสารเมลามีนในนมที่ประเทศ จีนเป็นเหตุให้เด็กเสียชีวิต 4 ราย มีเด็กล้มป่วยกว่า 60,000 คน อีก 150 รายเกิดอาการไตวาย ทำให้หลายประเทศที่นำเข้าสินค้าจากประเทศจีน  ต้องเร่งตรวจสอบสารเมลามีนใน ผลิตภัณฑ์นมผงและสินค้าที่เกี่ยว ข้องอย่างเร่งด่วน ซึ่งนั่นก็รวมถึง ประเทศไทยด้วย</p>
<p>หลายคนคงเคยได้ยินชื่อ เมลามีน (Melamine) กันมาบ้าง แล้ว ด้วยคุณสมบัติที่ทนความร้อน เมื่อนำมาทำปฏิกิริยากับฟอร์มาลดีไฮด์ (formaldehyde) จะได้พลาสติกทนความร้อน จึงนิยมนำมาผลิตพลาสติกเป็นภาชนะที่รู้จักกันในนามของ ชาม จานเมลามีน รวมทั้ง ถุงพลาสติก ยาฆ่าแมลง น้ำยาดับเพลิง น้ำยา ทำความสะอาด นอกจากนั้นยังใช้เป็นกาวสำหรับอุปกรณ์ที่ใช้ภายนอกอาคาร เนื่องจากมีคุณสมบัติกันน้ำ รวมถึงใช้เคลือบผิวเฟอร์นิเจอร์เพื่อทนต่อสารเคมี</p>
<p>แต่ที่นำเมลามีนมาปนเปื้อนในนมนั้น ดร.ปิยวิทย์ คุ้มพงษ์ นักวิจัย กลุ่มเทคโนโลยีฟิล์มพลาสติกศูนย์เทคโนโลยีโลหะและวัสดุแห่งชาติ สวทช. ให้ความรู้เกี่ยวกับสารเมลามีนว่า เมลามีนเป็นสาร เคมีประเภทไตรอะซีน (triazine) ซึ่งเป็นสารตั้งต้นในการผลิตอะมิโน เรซินและพลาสติก มีลักษณะ เป็นผลึกของแข็งสีขาว มีความหนาแน่นสูงกว่าน้ำ</p>
<p>เมลามีน มีสูตรโครงสร้างทางเคมี C3H6N6 (1,3,5 -Tria zine -2,4,6 -Triamine) สามารถ ละลายน้ำได้เล็กน้อย มีลักษณะเป็นคอลลอยด์เช่นเดียวกับน้ำนมสด นอกจากลักษณะทางกายภาพที่เหมือนกับนมผงมากจนแทบแยก ไม่ออกแล้ว เมลามีนบริสุทธิ์ยังมีองค์ประกอบของไนโตรเจนสูงถึง 66.67% คิดเป็นปริมาณโปรตีนได้ 416.66% ดังนั้นเมื่อนำเมลามีนมาผสมในน้ำนมหรือนมผงก็ทำให้ ผลการตรวจพบเปอร์เซ็นต์ไนโตร เจนสูงขึ้นด้วย จึงทำให้เข้าใจผิดได้ ว่าผลิตภัณฑ์ที่ตรวจนั้นมีโปรตีนสูง  คุณภาพดีได้มาตรฐานด้วย เนื่อง จากการตรวจหาปริมาณโปรตีนในปัจจุบันยังไม่สามารถทำได้โดยตรง แต่จะทำทางอ้อมด้วยการตรวจ หาปริมาณไนโตรเจนแทน เนื่อง จากโปรตีนมีไนโตรเจนเป็นองค์ประกอบเช่นเดียวกัน ตรงนี้คาดว่าเป็นสิ่งที่ผู้ผลิตจงใจ การใส่ เข้าไปลักษณะเช่นนี้เรียกว่าการปลอมปน สำหรับเมลามีนที่เป็นส่วนประกอบของผลิตภัณฑ์ ไม่ว่าจะเป็นเค้กหรือขนมต่าง ๆ ด้วย ความไม่รู้หรือไม่ได้ตั้งใจ จะจัดว่าเป็นสารปนเปื้อน</p>
<p>โดยปกติแล้ว คนที่ทำงาน เกี่ยวข้องกับสารเคมีจะต้องระวังค่า LD50 ซึ่งเป็นค่าวัดความรุน แรงของสารเคมี สำหรับ LD50 ของเมลามีนจะอยู่ที่ 3,100-3,300 มิลลิกรัมต่อน้ำหนักตัว 1 กิโลกรัม  ของผู้ที่ได้รับต่อวัน นั่นคือ 3,000 มิลลิกรัม เท่ากับ 3 กรัมต่อน้ำหนัก ตัว 1 กิโลกรัม ถ้าน้ำหนัก 50 กิโลกรัม ต้องกินเข้าไปถึง 150 กรัมต่อวัน ถึงจะเป็นอันตราย ด้าน ฝั่งยุโรปปริมาณที่ยอมรับได้อยู่ที่ 0.50 มิลลิกรัมต่อน้ำหนักตัว 1 กิโลกรัม แต่ในฝั่งอเมริกาอยู่ที่ 0.63 มิลลิกรัม ต่อน้ำหนักตัว 1 กิโลกรัม เช่น เด็กน้ำหนักตัว 30 กิโลกรัม สามารถรับได้ 18.9 มิลลิ กรัมต่อวัน ถือว่าไม่เป็นอันตราย</p>
<p>“สารเมลามีนนั้น ไม่ก่อให้เกิดการระคายเคืองตาและ ไม่จัดเป็นสารก่อมะเร็ง แต่หากเมลามีนรวมตัวกับกรดไซยานิว ริก ซึ่งเป็นสารที่มีโครงสร้างทางเคมีคล้ายเมลามีนจะเกิดผลึกที่ไม่ละลายน้ำ เมื่อกินเข้าไปจะไปฝังตัวอยู่ในไตทำให้เกิดเป็นนิ่วใน  ไตหรือนิ่วในกระเพาะปัสสาวะ  จนอาจทำให้เกิดอาการไตวายได้”</p>
<p>สาเหตุที่สามารถนำเมลามีนมาใช้ในอาหารได้ เนื่องจากมี การจดสิทธิบัตรการใช้เมลามีน ในสัตว์เคี้ยวเอื้อง เป็นอาหารสัตว์โดยตรงด้วยเหตุผลเดียวกัน คือ มีไนโตรเจนมาก ที่นี้จัดเป็นนอนโปรตีนไนโตรเจน (Non-Protein Nitrogen : NPN) คือ เป็นไนโตรเจนที่ไม่ได้มาจากไนโตรเจน โดยตรง เพราะกระเพาะของสัตว์ เคี้ยวเอื้องมีแบคทีเรียช่วยย่อย ต่างจากมนุษย์ที่ไม่สามารถย่อยสารพวกเซลลูโลสหรือไฟเบอร์จากธรรมชาติได้ จึงมีสิทธิบัตรใช้เมลามีนเป็นอาหารสัตว์ แต่ว่าหลังจากการศึกษาเพิ่มเติมพบว่าสัตว์ไม่สามารถย่อยได้ทัน สุดท้ายจะไปตกค้างอยู่ในสัตว์ พวกนี้</p>
<p>ทำให้ สำนักงานอาหารและยาของประเทศสหรัฐอเมริกา (FDA) ห้ามใช้เมลามีนเป็นอาหาร พวกสัตว์เคี้ยวเอื้อง แต่ว่าตระกูลที่คล้ายกับเมลามีน อย่าง กรดไซยานิวริก สามารถใช้ได้เพราะกรดไซยานิวริกย่อยได้ง่ายกว่า แต่มีระบุไว้ชัดเจนว่าใช้เป็นอาหารสัตว์ได้เฉพาะสัตว์เคี้ยวเอื้องเท่านั้น และที่สำคัญต้องระบุให้ชัดเจนว่ามีนอนโปรตีนไนโตรเจน เป็นส่วนประกอบอยู่ทั้งหมดเท่าไร โดยห้ามนำมาเป็นอาหารสัตว์ชนิดอื่น ๆ เช่น หมู ปลา เป็ด ไก่ เด็ดขาด</p>
<p>ด้านการบริโภค ดร.ปิย วิทย์ กล่าวว่า อันดับแรก คือ ต้องทำความเข้าใจก่อนเพราะบางคนคิดว่ามีเมลามีนเฉพาะในนมผง ที่ประเทศจีนมีการสุ่มตรวจปรากฏว่ามีในนมสดด้วยแต่พบเพียง 10 เปอร์เซ็นต์ของตัวอย่าง ฉะนั้นต้องมองไปถึงนมทุกประเภท  ปัจจุบันคนนิยมบริโภคนมถั่วเหลือง กันมากขึ้น ยิ่งในช่วงกินเจ หลายคนไม่เข้าใจว่าทำไมนมถั่วเหลืองจึงต้องมีสูตรเจ เพราะความจริงแล้วนมถั่วเหลืองจะมีการผสมนมผงเข้าไปด้วย ผู้บริโภค จึงต้องอ่านฉลากให้ดี</p>
<p>หากย้อนไปในปี ค.ศ. 2004-2007 ในส่วนผสมที่เป็นอาหารสัตว์ เมื่อสุนัขและแมวกิน  เข้าไปแล้วมีการตายเกิดขึ้นเป็นจำนวนมาก จากการตรวจพบว่ามีเมลามีนในโปรตีนจากข้าวสาลี และพบเมลามีนรวมกับกรดไซยานิวริกในโปรตีนข้าวเข้มข้น ซึ่งพบประมาณ 2-80 กรัมต่อน้ำหนักผลิตภัณฑ์ 1 กิโลกรัมในอเมริกา ใต้พบเมลามีนในโปรตีนข้าวโพด เป็นผลให้สหภาพยุโรป มีการควบคุมตรวจเช็กทุกลอตไม่เฉพาะ สารที่กล่าวมาแต่รวมไปถึง ผลิต ภัณฑ์อาหารที่ทำจากข้าวโพด โปรตีนจากถั่วเหลือง และรำข้าว เพื่อเป็นการป้องกันก่อนที่จะเกิดปัญหาขึ้น แต่ยังไม่มีรายงานว่าพบในอาหารคน ฉะนั้นสิ่งที่ควรระวัง คือ อะไรก็ตามที่ไม่ใช่เนื้อสัตว์แล้วบอกว่าใช้แทนโปรตีนได้และมีโปรตีนสูงพึง ระวังก่อนที่จะเลือกบริโภค</p>
<p>ส่วนภาชนะเมลามีน ถ้าใช้เมลามีนซึ่งเป็นวัตถุดิบที่มีคุณ ภาพ มีความบริสุทธิ์สูงอย่างน้อย 95 เปอร์เซ็นต์ มีการผลิตที่ได้มาตรฐาน ซึ่งมีการทำปฏิกิริยาเคมี ที่สมบูรณ์และถ้าใช้งานตามปกติที่อุณหภูมิห้องจัดว่าปลอดภัย แต่ต้องหลีกเลี่ยงการใส่อาหารที่มีฤทธิ์ เป็นกรดที่อุณหภูมิสูง โดยเปลี่ยนมาใช้ภาชนะที่เป็นเซรามิกแทนจะ ปลอดภัยกว่า</p>
<p>ส่วนจานชามเมลามีนที่ ราคาถูกผิดปกติ อาจผลิตจากเมลา มีนที่มีความบริสุทธิ์ต่ำ หรืออาจเป็นยูเรียฟอร์มาลดีไฮด์รวมทั้งมีกระบวนการผลิตที่ไม่ได้มาตรฐาน ทำให้การเกิดปฏิกิริยาไม่สมบูรณ์ ส่งผลให้สารเคมีที่ใช้ในการผลิตตกค้างและอาจปนเปื้อนอาหารในปริมาณมากเกิน กว่าที่จะยอมรับได้</p>
<p>“มีผลิตภัณฑ์หลายยี่ห้อที่ส่งไปให้สำนักงานคณะกรรมการอาหารและยา (อย.) ตรวจสอบเพื่อยืนยันว่าไม่มีสารเมลามีนผสม อยู่ โดยหลักการสุ่มตัวอย่างไม่ ควรให้ผู้ผลิตส่งไปให้ตรวจ แต่ เจ้าหน้าที่ควรจะสุ่มตรวจเอง เพราะ ผู้ผลิตอาจจะคัดผลิตภัณฑ์ที่ดีมาให้ตรวจได้ ส่งผลในแง่ของวิทยาศาสตร์ที่ว่าความน่าเชื่อถือจะมีหรือไม่ หน่วยงานที่รับผิดชอบ น่าจะเป็นผู้สุ่มตรวจและมีการ สุ่มตรวจอยู่เรื่อย ๆ และไม่ควรจำกัดอยู่ที่นม อย่ารอให้เกิดเรื่องแล้วค่อยสนใจเพื่อจะช่วยให้ผู้บริโภคมีความมั่นใจและ ปลอดภัยจากสารพิษเหล่านี้มากยิ่งขึ้น”</p>
<p>ฉะนั้นผู้บริโภคจำต้อง พยายามหาความรู้ด้วยตนเองด้วย เมื่อได้ข่าวสารแล้วอย่าเพิ่งตื่นตัวหรือส่งข่าวต่อแต่ควรตรวจเช็กความ ถูกต้องเสียก่อน ควรเฝ้าระวังแต่อย่าถึงกับระแวง ควรทาน อาหารที่มีความหลากหลายเพื่อลดความเสี่ยงในการรับสารพิษตัวใดตัวหนึ่งใน ปริมาณมาก และ ควรดื่มน้ำมาก ๆ</p>
<p>ส่วนผู้ผลิตน่าจะช่วยได้มากถ้าใส่ใจถึงคุณภาพของสินค้าที่ผลิตเพราะจะทราบดี ว่าวัตถุดิบที่นำมาทำคืออะไร คุณภาพเป็นอย่างไรและมาจากแหล่งใด อีกอย่างหนึ่ง คือ เรื่องฉลากสินค้าควรระบุรายการให้ชัดเจน อีกทั้ง การให้ข้อมูลที่ถูกต้อง ทันเวลา และน่าเชื่อถือ จากหน่วยงาน ที่เกี่ยวข้องน่าจะช่วยผู้บริโภคได้มาก</p>
<p>ที่สำคัญ อย่าใช้ภาชนะผิดประเภทพลาสติกที่ไม่ใช่ชนิดใช้กับอาหาร ไม่ทนความร้อนไม่ควรใช้กับอาหาร อ่านฉลากผลิต ภัณฑ์ให้เข้าใจและหาความรู้เพิ่มเติมเพื่อคุณภาพชีวิตที่ดี ดร.ปิย วิทย์ กล่าวด้วยความเป็นห่วง</p>

<p align="justify">For most of us, acne seems to be the bane of our existence. And  it is not totally unfounded. Acne, nowadays, is the most prevalent and most  common medical condition affecting more than 60 million people in the United  States alone.</p>
<p><strong>What Are The Causes Of Acne?</strong><br />
Here are the most  common causes of acne that we have gathered for you. Hormones: the number one  cause of acne is the production of sex hormones, known as androgens, that begins  at puberty. This is why the majority of acne sufferers are adolescents and  teens. Hormones are also responsible for acne flare-ups during menstruation and  pregnancy. When the sebaceous gland is stimulated by androgens, it produces  extra sebum. In its journey up the follicle toward the surface, the sebum mixes  with common skin bacteria and dead skin cells that have been shed from the  lining of the follicle. While this process is normal, the presence of extra  sebum in the follicle increases the chances of clogging — and acne.  <strong>STRESS</strong> can also cause the production of hormones, such as cortisol, which  can aggravate acne. Stress brings upon different hormone levels. With hormone  changes the body prompts the skin's oil glands to enlarge, secreting more oil  which causes white heads, black heads, and pimples. Oily or heavy makeup: Heavy  makeup clogs the pores and oily makeups add more oil, which only adds to the  problem if oily skin already exists. Cosmetics, especially certain moisturizers,  foundations and pomades contain lanolin, petrolatum, vegetable oils, butyl  stearate, lauryl alcohol and oleic acid. Certain medications and steroids:  Vitamins are good for your body, But an excess of vitamins B1, B6 and B12 can  cause acne flare-ups. These vitamins are good for the skin, but avoid overuse.  Diet: For some people, a diet that is high in refined carbohydrates and sugars  can actually aggravate their acne. Over abrasive cleansing: Harsh exfoliators  can damage skin and spread infections. Picking and squeezing: This can actually  send the infection deeper into the skin and can cause scarring. Environmental  irritants such as high humidity and pollution. High humidity actually causes  swelling of skin. Pollution is of course a very big “No No” to good health, not  just skin. Genes of family members who are also acne sufferers. Acne is  inherited and severe cases known as cystic acne usually come from heredity. If  you have a very persistent case of acne, it is wise to consult your doctor.</p>

In the United States alone,  nearly 60 million people are infected with acne, while 85 percent of teenagers  suffer from it.<br />
Skin experts and practitioners recommend several natural acne  treatments. Here are some that we have compiled just for you.</p>
<p align="justify">Wash your face twice a day (twice at most) with gentle and  unperfumed cleanser. Avoid using soaps as they contain harsh chemicals and  ingredients that can damage your skin.</p>
<p align="justify">Avoid touching your face. Stop putting your hands on your face!  Also rubbing or bracing your chin is another common problem when thinking. Avoid  rubbing, touching, or scratching your skin with your hands. Your hands contain a  lot of bacteria that can cause acne flare-ups. It is probably one of the most  difficult things to avoid since much of the hand to face contact throughout the  day we are unconscious of. Make it a habit to avoid hand contact and be  conscious of it during the day to avoid bacteria.</p>
<p align="justify">Avoid the temptation to pick, prick and squeeze your acne. This  will send the infection deeper into the skin and can cause severe scarring.</p>
<p align="justify">For mild to moderate acne, you can use over-the-counter topical  ointments, solutions, lotions or gels that contain benzoyl peroxide, salicylic  acid or azelaic acid as an alternative to benzoyl peroxide. Benzoyl peroxide is  best at killing <em>P. acnes</em> and may reduce oil production. Resorcinol,  salicylic acid, and sulfur help break down blackheads and whiteheads. Salicylic  acid also helps cut down the shedding of cells lining the follicles of the oil  glands.</p>
<p align="justify">Topical antibiotic solutions and lotions can also be  applied.</p>
<p align="justify">Tea tree oil is a natural antibiotic and antibacterial agent  and has a drying effect on the skin. It keeps the P. Acnes bacteria at bay along  with decreasing facial oiliness, which makes this oil a worthwhile  investment.</p>
<p align="justify">Sulfur helps to heal existing blemishes by unblocking  pores.</p>
<p align="justify">Alpha or Beta Hydroxy Acids (AHA or BHA) works by keeping the  skin exfoliated. Glycolic acid, the most well known of the bunch is a useful  adjuvant therapy for mild acne. Mandelic acid, a lesser known one, but one that  combines the keratolytic properties of glycolic acid, with natural antibacterial  properties that help reduce the presence of <em>P. acnes, </em>may be considered a  more effective alpha hydroxy acid in treating acne lesions. It is also much less  irritating than glycolic acid, a factor that may be very important to those with  sensitive skin who are unable to use other agents such as Retin-A, benzoyl  peroxide, sulfur, or salicylic acid. It is effective in treating mild cases of  acne on its own, and can be used successfully with other therapies on moderate  acne.</p>
<p align="justify">These treatments are usually effective for mild to moderate  acne problems only. The effectiveness usually takes several weeks or  approximately three to six months to be noticed. For more severe and serious  acne conditions, do not hesitate to consult your dermatologist.</p>

<blockquote><p>The study found that among those who never smoked, Asians living in Asia (not those in the United States) and African-Americans had higher rates of illness and death from lung cancer than did people of European descent.</p>
<p>A puzzling and troubling finding is relatively high rates of lung cancer among nonsmoking women in parts of Pacific Rim countries.</p>
<p>“A plausible hypothesis is that the cooking fumes given off by woks contain all kinds of carcinogens,” Dr. Thun said, adding that the most likely culprit was cooking oil, which vaporizes at high temperatures. But he said the problem needed more research.</p></blockquote>
<p>Dr. Michael J. Thun, the lead author of the study and the head of epidemiologic research for the American Cancer Society.  The same Dr. Thun that Lambasted Enstrom/Kabat.  I don't see him saying the debate is over.  I don't see him saying anything close to being conclusive. <p align="justify">#1. You will need one egg, a heaping tablespoon of mayonnaise  and a tablespoon of honey. Mix this together thoroughly with a fork and apply  sparingly to the skin. Allow this to dry and then wipe off with a warm  washcloth.</p>
<p align="justify">#2. You will need 1 tbsp. yeast, 1 tbsp. baking soda and 1  tbsp. water. Mix together ingredients and apply on face allowing it to sit for  15 minutes before rinsing it with warm water.</p>
<p align="justify">#3. You will need 1 tbsp. of gram flour, 1/4 tsp. orange peel  powder, 1 tbsp. beaten yogurt and 1 tsp. olive oil. Mix ingredients well and  apply paste on your face and neck allowing it to dry. Rinse with warm water  first then cold water.</p>
<p align="justify">#4. Slices of cucumbers applied to the eyes refresh and relieve  puffiness and they are great cleansers when sliced and rubbed over your face.  The same holds true for slices of fresh tomatoes.</p>
<p align="justify">#5. You will need one-fourth tsp. of lime juice stir to mix  with one tsp. of milk and cucumber juice. Apply on the face and neck and wash  off after 14 minutes. This cleanses and purifies pores of skin.</p>
<p align="justify">#6. Make a paste by mixing papaya, honey, milk and milk powder.  Apply on the face and let sit for a few minutes then wash off.</p>
<p align="justify">#7. You will need 1/2 tsp of rose water mixed with 1 tsp of  honey. Gently apply to the face. Leave it for about 15-20 minutes and then wash  off with tepid water. Honey moisturizes and soothes dry skin, and is gentle  enough for daily use.</p>

<content:encoded><![CDATA[<p><a href="http://secretwave101.files.wordpress.com/2008/10/image_087.jpg"><img class="size-medium wp-image-423 alignleft" title="image_087" src="http://secretwave101.wordpress.com/files/2008/10/image_087.jpg?w=300" alt="" width="300" height="240" /></a>I suppose the title of this entry could just as easily refer to a patient who reached a century of life, but I'm actually musing on the centenarian <em>building </em>where I'm working this month.</p>
<p>I'm sentimental.  I love history - loved it more than biology in undergrad.  So a building like this place carries whispers of the past into my psyche every day I arrive.  I bask in a steady breeze of stories - known and unknown - each day when I walk through the back door.  New Yorkers often explain the love of their city by describing the enormous number of engaging cultural things available to do...even if they never do them.  This building offers the same thing, but in the currency of stories.</p>

<p>You could spend months here just learning about all the things this building has seen.  You can ponder the tree stump near the front door where a guy blew himself up with dynamite while his wife was inside for an appointment.  Or think about the years when the building served as the town's hospital, complete with an ER, lab, mortuary and second floor (since removed).  Also drifting through the single hall and 4 exam rooms are the ghosts of the doctors who pioneered the practice, and the dogged support staff that kept this place running through years of desolation, poverty, isolation and depression.  This building survived storms, heat, wind, rain and over a hundred hunting seasons.</p>
<p>Consider the relative youth of even the oldest parts of our culture:  We're tykes on the world stage.  The oldest American culture - Native American - has been essentially glossed and veneered over by our exuberant European ancestors so that our main cultural heritage in America references the late 1700's.  I suppose if you're really trying to push the date back, you could reference 1492 and Columbus, but really, the American Experiment started in 1776.  This makes us babies among world cultures.  Embryos, practically.</p>
<p><a href="http://secretwave101.files.wordpress.com/2008/10/image_092.jpg"><img class="size-medium wp-image-421 alignleft" title="image_092" src="http://secretwave101.wordpress.com/files/2008/10/image_092.jpg?w=300" alt="" width="300" height="240" /></a>So a building like this is one of the few material references to times and people long dead or moved away.  We are a transient culture, and our history is still too new to be truly memorable.  We need a good couple thousand more years to really figure out who we are, what we're about.  Until then, some of us hold on to places like this: a lonely building filled to the weakening rafters with wisdom, loss and joy.</p>

<content:encoded><![CDATA[<p>i wanted to jot this down for my own records! :) my little girl is just growing and learning by leaps and bounds these days!!  at this rate we won't 'need' to add many more signs!</p>
<p>our morning game we play-when i walk in her room she says' where's kiki?' and i say i don't know in a real sing song voice. then i have to find kiki.he is either on the floor or she is hiding him! when kiki appears he gets smothered in exaggerated big kisses with sound effects and all-by erika!!! and me! it's fun!! and she only does it with me!! awww if daddy gets her in the morning she just puts her arms out right away to get picked up!</p>
<p> then once on the change table she wiggles her shoulders and says- where's dada? and i tell her where he is.then she goes down the whole list!! where's pop pop? where's nana? and she tries to say the boys names too.then i ask her where's mama?and she points at me and says right there! pure sweet fun i tell ya!</p>
<p>when she sees the camera she says CHEEEEEESE! when she hears a squeaky noise she runs to it and says shoes? shoes?  HAHA!! don't dare put a pair of shoes on her that don't squeak-she will be pissed off!! it's great advertising when out and about! ;) she has started to try and dress herself and she can sucessfully put shoes on-at times! lol</p>

<p>she also tries super hard to say lukas and jakob but they come out kind of like kuku and kayko.<br />
and she pretty much will say any word you ask her to if she is in the mood! i think her verbal skills are amazing for 17m old! i think the 'average' for this age is 15 words!!!</p>
<p>she still only has 6 teeth,but boy oh boy that doesn't stop her from competing with daddys appetite! woa baby! let's see...some of the more amazing feats accomplished in a single meal: an entire pb&#38;j sandwich,banana and grapes.another time was 7 french toast sticks(no syrup)and a banana.another time was 2 slices(cut up)of cheese pizza. i dunno where it goes!!</p>
<p>she is also fully weaned and we did it on HER time! i am glad to be able to say that.she was so ready and it made it so much easier for me! we made it to 17months!! that's 11 months more than i ever imagined i would breastfeed her!! i wasn't prepared for the depression during the weaning though. i never thought to put 2 and 2 together...when you nurse your body releases the feel good hormones so of course when you STOP they are no longer there! and well,i need all the feel good hormones i can get my brain on! haha! it was really really bad! almost as bad as experiencing PPD all over again.once some of my online friends pointed out this was most likely the cause,i felt better.it's always nice to know WHY you feel like a hormonal pc of crap! i was living in a dark depressed weepy world for a good 2 1/2 weeks. i can't even begin to describe how hard it was for me.i know it had to have been hard on my family too.they also suffered. :( but it's over now and things are back to normal....um....yea.....whatever THAT is! oh-and my hair is growing again!! yippee!!! and i can take percoset 2-3 times a day!! yippee yippee!! and daddy can put erika to bed at night when he is home!! woohoo! and i can start stronger meds this week to help with all the other mood swings and depression! lmao i've been off for awhile now and yea,i am ready to start again.</p>
<p>she has an incredibily strong bond with lukas.it's understandable as they are so close in age and together all day while jakob is in school. but i am trying to involve jakob into her 'circle' more too.i think it's important for both of them.lukas gets erika into soooo much mischief!! omg!</p>
<p>just last night(since it is 3am here!)lukas pushed her off of her ride on toy and she cried so hard! i picked her up and consoled her and she cried for a couple minutes.then i said-do you want to go brush your teeth? she stopped crying instantly and said YEA!! it was hysterical!! even jakob commented from the other room-did that just get her to stop all that crying?? woohoo</p>

<content:encoded><![CDATA[<p>All immunosuppressive regimens used in organ transplan­tation increase the risk of infection and malignancy.<br />
<strong>Infection</strong><br />
Transplant recipients receiving immunosuppressive therapy are at high risk from opportunistic infection, especially by viruses. Opportunistic infection is a potential problem in all transplant recipients, but those receiving aggressive immunosuppressive therapy after liver, heart, lung and small bowel transplantation are most at risk. Chemoprophylaxis is important in high-risk recipients.<br />
<strong>Bacterial infection</strong><br />
The risk of bacterial infection is highest during the first month after transplantation. Transplant recipients are, like all patients undergoing major surgery, at risk of bacterial infections in the wound, respiratory tract and urinary tract. It is standard practice to give a broad-spectrum antibiotic to cover the perioperative period as prophylaxis against wound infection and possible bacterial contamination of the donor organ. The risk of bacterial infection is greatest in transplant recipients who are critically ill before or after surgery and are in the intensive care unit with indwelling catheters and lines. After recovery from surgery, the risk of bacterial infection is much reduced. Tuberculosis is a concern in patients who have previously had rnycobacterial infection and in patients from the Indian subcontinent, and it is usual to give immunochemoprophylaxis to these individuals for a period of 6—12 months after transplantation.<br />
<strong>Viral infection</strong><br />
The risk of viral infection is highest during the first 6 months after transplantation and the most common problem is CMV infection. Most adults (&#62; 80 per cent) develop CMV infection when they are children and have acquired immunity, as evidenced by the presence of IgG antibodies to CMV Following organ transplantation, CMV disease may arise because of reactivation of latent infection or because of primary infection that can be transmitted by an organ from a CMV-positive donor. The recipients at most risk from CMV infection are those who are CMV seronegative and receive an organ from a CMV-seropositive donor. Matching seronegative donors with seronegative recipients is an effective strategy for reducing the risk of CMV infection but is not very practicable. CMV disease typically presents with a high swinging fever, lethargy and leucopenia. The severity of the disease is variable and the clinical picture depends on the organ system most affected. It may present as:<br />
•  pneumonia;<br />
• gastrointestinal disease;<br />
• hepatitis;<br />
• retinitis;<br />
•encephalitis.<br />
Severe CMV disease is potentially fatal. Prophylaxis for CMV consists of passive immunisation using hyperimmune irnmunoglobulin or, more commonly now, administration of antiviral agents in the form of aciclovir or ganciclovir. A diagnosis of active CMV infection can be confirmed by serology, detection of CMV antigenaemia, CMV culture, PCR to detect viral DNA and histological examination of biopsy material. Treatment is with ganciclovir and is more effective when given pre-emptively on the basis of increased viral load as judged, for example, by quantitative PCR analysis for CMV<br />
Herpes simplex virus (HSV) infection is common after transplantation and is usually due to reactivation of latent infection. It causes rnucocutaneus lesions around the mouth and sometimes the genitalia. These usually respond to topical treatment with acyclovir, but in severe cases systemic antiviral therapy is needed. Disseminated HSV infection is rare. Varicella zoster infection occurs more frequently in transplant patients and should be treated with systemic antiviral therapy.<br />
<strong>Protozoal infection</strong><br />
Pneumocystis carinii is the most important protozoal infec­tion after transplantation. It occurs during the first few months and presents with respiratory symptoms. The diagnosis is made by examination of bronchoalveolar lavage fluid or lung biopsy material for evidence of parasite infec­tion. Prophylaxis with co-trimoxazole is highly effective and is usually continued for 6 months after transplantation.<br />
<strong>Fungal infection</strong><br />
Invasive fungal infections are uncommon in renal transplant recipients but infection with Candida or Aspergillus is more common after other types of solid organ transplantation. Fungal infection usually occurs in the first 3 months after transplantation, and early diagnosis and aggressive treatment are essential to avoid fatal infection.<br />
<strong>Malignancy</strong><br />
After transplantation there is an increased risk of developing certain types of malignant disease, especially those turnouts where viral infection plays an aetiological role. The increased risk of malignancy is particularly high for skin cancer and non-Hodgkin’s lymphoma. Most of the skin cancers seen are squamous cell carcinomas, but basal cell carcinoma and malignant melanoma are also more common. The risk of skin cancer after transplantation rises with age and with exposure to sunlight, and it has been predicted that 50 per cent of transplant patients will develop a skin malignancy by 20 years. Patients must be warned of this risk before they undergo transplantation and advised to use an effective sun-screen. They should undergo regular review of their skin to detect malignancy and when such lesions occur they must be treated promptly and aggressively.<br />
The risk of developing non-Hodgkin’s lymphoma is significantly increased after transplantation. This type of malignan­cy is classified as post-transplant lymphoproliferative disease (PTLD). Most of them are Epstein—Barr virus-induced B-cell turnouts. The overall risk of PILD is around 1 per cent per annum and is highest in patients who have received aggressive immunosuppression. PILD may occur in lymph nodes or at extranodal sites such as the gastrointestinal tract, lung, liver or in the transplanted organ. If PILD is identified at an early stage reduction or cessation of immuno­suppressive therapy may cause disease regression and result in a cure. When advanced it is usually fatal. Both disseminated disease and central nervous system (CNS) involvement have a very poor prognosis.<br />
Transplant patients also have a 300-fold increased risk of developing Kaposi’s sarcoma, although this malignancy is still very uncommon after transplantation.</p>

In tracing the development of clinical immunosuppression, three different eras can be identified:<br />
• the early or precyclosporin era (from the early 1960s to the late 1970s);<br />
• the cyclosporin era (from the early 1980s);<br />
• the modern era (the 1990s).<br />
Until the discovery of cyclosporin in the late 1 970s, steroids and azathioprine were used for immunoprophylaxis. During the cyclosporin era the monoclonal antibody OKT3, directed against the CD3 molecule expressed on T-lymphocytes, also became available and was sometimes used along with cyclosporin based regimens as immunoprophylaxis (especially in North American centres) or to treat steroid resistant graft rejection. The 1990s can be regarded as the modern era of immunosuppression. Although cyclosporin is still the mainstay of many immunosuppressive regimens, several new agents have now been introduced. The place of cyclosporin has been challenged by tacrolimus and other important immunosuppressive agents have recently become available, including mycophenolate mofetil, rapamycin and monoclonal antibodies directed against the interleukin-2 (IL-2) receptor (CD2S). When considering immunosuppressive therapy for organ transplantation, the transplant surgeon is now faced with an increasing range of different agents from which to choose .<br />
<strong>Calcineurin blockers</strong><br />
Cyclosporin and tacrolimus, although structurally distinct, exert their principal immunosuppressive effect through the same pathway. Each of the two agents binds within the T-cell to a particular cytoplasmic protein or immunophilin (cyclosporin binds to cyclophilin and tacrolimus to FK binding protein). The resulting immunophilin—drug complex then blocks the activity of calcineurin (a phosphatase) within the cytoplasm of the T-cell. Calcineurin plays a critical role in facilitating the transcription of IL-2, the main T-cell growth factor, and other cytokines after T-cell activation. By blocking cytokine synthesis, cyclosporin and tacrolimus exert a potent immunosuppressive effect. The two agents share a number of side effects, the most notable of which is nephrotoxicity. Cosmetic side effects may beparticularly distressing for the patient. The calcineurin blockers have a relatively small therapeutic window, and monitoring of whole blood drug levels is an important guide to optimal therapy. If, after renal transplantation, the graft fails to function immediately because of acute tubular necrosis, calcineurin blockers may be withheld temporarily to avoid the risk of drug-induced nephrotoxicity. The relative efficacy of cyclosporin and tacrolimus in pre­venting graft rejection is broadly comparable, and the choice between the two agents is dependent on the preference of the transplant unit and on individual patient tolerance to the different side effects of the two agents.<br />
<strong>Antiproliferative agents</strong><br />
Lymphocytes are among the most rapidly proliferating cells in the body, and lymphocyte proliferation and clonal expansion is an integral part of the immune response to an allograft. The antiproliferative agents available for immunoprophylaxis are azathioprine and mycophenolate. Azathioprine is converted in the liver to its active metabolite 6-mercaptopurine which blocks purine metabolism and thereby inhibits cellular proliferation. Mycophenolate is a recently introduced antiprolifera­tive agent that, after ingestion, is converted to its active metabolite, mycophenolic acid. It inhibits the enzyme inosine monophosphate dehydrogenase that is the rate limiting enzyme in the de novo pathway of purine nucleotide synthesis. Because lymphocytes do not have a salvage pathway for purine synthesis, their ability to proliferate is selectively impaired.<br />
<strong>Steroids</strong><br />
Steroids have always been an important component of immunoprophylactic regimens. Glucocorticoids are potent anti-inflammatory agents and have wide-ranging effects on the immune response. Because of their numerous and well-known side effects, many centres attempt to withdraw gradually steroids from patients who have stable graft function at around 1 year after transplantation, but this sometimes precipitates a rejection episode and necessitates continued treatment with steroids.<br />
Antibody therapies<br />
Many North American transplant units also include antilym­phocyte antibody preparations as part of their immunopro­phylaxis — so-called ‘quadruple therapy’. A polyclonal antilymphocyte preparation (ALG or ALS) may be used or, alternatively, a monoclonal antibody directed against CD3 or CD25 (IL-2 receptor) onTcells may be given. In Europe, the use of antibody preparations is more common after heart transplantation, where irreversible rejection is usually synonymous with death of the patient. Antibody therapy is often used in renal transplantation for patients who are thought to be at particular risk from graft rejection, for exam­ple highly sensitised and second- or third-time graft recipients.<br />
<strong>Rapamycin</strong><br />
Rapamycin is a newly discovered immunosuppressive agent that was isolated from a fungus found on Easter Island. Like tacrolimus, it is a macrolide which binds within the I-cell to FK binding protein. However, the mode of action of rapa­mycin is completely different to that of both cyclosporin and tacrolimus. It acts by interfering with intracellular signalling from the IL-2 receptor and arrests I-cell division in the G1 phase. Rapamycin and cyclosporin therefore act at different stages in I-cell activation and their immunosuppressive effects are synergistic.<br />
<strong>Immunosuppressive regimens</strong><br />
When selecting an immunosuppressive regimen some of the important points to bear in mind are as follows.<br />
• The challenge is to provide levels of immunosuppression that are sufficient to protect the graft from rejection with­out exposing the recipient to excessive risk from infection and malignancy as a result of nonspecific immunosuppression.<br />
•      Immunoprophylaxis is started at the time of transplanta­tion and continued indefinitely (as maintenance therapy), although the requirement for immunosuppression is highest in the first few weeks after transplantation when the risk of acute rejection is greatest.<br />
• Individual immunosuppressive protocols vary somewhat but almost all use a combination of immunosuppressive agents. All include a calcineurin blocker (cyclosporin or tacrolimus) as the main agent and this is most often given along with an antiproliferative agent (azathioprine or myco­phenolate mofetil) and steroids — so-called ‘triple therapy’. Less often, a calcineurin blocker is used with an antiproliferative agent alone or with steroids alone (dual therapy).<br />
• A few renal transplant units use monotherapy with a cal­cineurin blocker and then add other agents only if needed to treat rejection. At the other end of the spectrum, some units, particularly those in North America, advocate the use of antibody induction therapy (monoclonal or polyclonal preparations) followed by a calcineurin blocker, an anti-proliferative agent and steroids (quadruple therapy).<br />
• Many renal transplant units reserve the more intensive antibody induction protocols for recipients judged to be at increased risk of graft rejection (e.g. highly sensitised recipients and grafts with a poor HLA match).<br />
• The principles of immunoprophylaxis are similar for all types of organ transplantation. However, after thoracic organ transplantation there is a tendency to use more intensive immunosuppression than for kidney transplantation, in part because loss of a heart or lung graft from rejection almost inevitably culminates in death. Interestingly, liver grafts seem less susceptible to graft rejection, for reasons that are still unclear.<br />
• The place of the newer immunosuppressive agents, such as rapamycin and anti-CD2S monoclonal antibodies, still remains to be established but recent trials have shown promising results.<br />
Acute rejection during the first 6 months of transplantation occurs in around 20—40 per cent of transplant recipients. Fortunately, the majority of acute rejection episodes responds to a short course of high-dose steroid therapy. This can be given as three daily intravenous doses of methylprednisolone (0.5—1 g/dose) or a several-day course of oral prednisolone (e.g. 200 mg tapering to 20 mg). If the response to treatment is inadequate or if acute rejection recurs it can often be treated successfully by recourse to antilymphocyte antibody therapy and/or switching from cyclosporin to tacrolimus (or vice versa).</p>
